Insiders Feedback: Create PR interactive form doesn't work when changing chat windows

Feature:
trying the interactive form for creating a new pull request. I can see it if I only keep that single chat open the whole time (while it is presenting the widget to the user but if I am working on different chat it won't show up when I returned to the original chat)

Steps:

1. ask chat A to open an issue in the vscode agents window
2. go to chat B in the vscode agents window
3. go back to chat A once it finishes
4. 🐛 bug now shows: it says it presented to user with an interactive modal but there is nothing there
